3 SELECT MED INTRODUCTION INTRODUCTION Included in this directory is a comprehensive list of participating providers and facilities. Use it to determine if a provider or facility is participating on your SelectHealth SM plan. You can also determine whether a provider is a Primary Care Provider (PCP), a Secondary Care Provider (SCP), or an ancillary provider. PRIMARY CARE PHYSICIAN OR PRIMARY CARE PROVIDER (PCP) A Primary Care Physician or Primary Care Provider is a general practitioner who attends to your common medical problems and provides preventive care and health maintenance. You are not required to choose a PCP, but seeing one provider routinely will help establish a relationship that can improve your healthcare. SelectHealth has classified the following types of physicians and providers, and their associated physician assistants and nurse practitioners, as PCPs: Certified Nurse Midwives Family Practice Geriatrics Internal Medicine Obstetrics and Gynecology (OB/GYN) Pediatrics Primary care providers have availability (or make certain a provider of comparable specialty is available) by telephone 24 hours, seven days a week for urgent/emergent medical situations to assure the availability of prompt, quality medical services. SECONDARY CARE PHYSICIAN OR SECONDARY CARE PROVIDER (SCP) SelectHealth categorizes any physician who is not classified as a Primary Care Physician as a Secondary Care Provder. Examples of an SCP include the following: Cardiologists; Dermatologists; Neurologists; Opthamologists; Orthopedic Surgeons; and Otolaryngologists (ENTs) You do not need a referral from your PCP to make an appointment with an SCP, or specialist. ANCILLARY PROVIDERS Participating ancillary providers are credentialed nonphysician clinicians that may provide either primary, secondary, or ancillary care. This type of provider includes physical therapists, licensed clinical social workers, audiologists, optometrists, and more. The ancillary provider section of this directory will indicate whether an ancillary provider is classified as a PCP or an SCP. MENTAL HEALTH PROVIDERS Mental health providers are psychiatrists, psychologists, and social workers who provide counseling. To verify your mental health benefits or to obtain help finding a mental health provider, please call our Behavioral Health Advocates SM at (Salt Lake area) or , weekdays, from 8:00 a.m. to 6:00 p.m. A referral is not required to see a mental health provider. EXTRA HELP FINDING A PARTICIPATING PROVIDER OR FACILITY Here are some additional resources that will help you find a participating provider or facility. Online Provider and Facility Search The online provider and facility search gives you fast, convenient information about providers participating on your plan. The most current list of providers (including Primary Care, Secondary Care, Ancillary Care, and Mental Health) is available at selecthealth.org. You can search using different criteria to help you find a provider or facility to meet your needs, including the following: Provider Information Gender Hospital affiliations Medical group affiliations Board certification Languages spoken by the practitioner or clinical staff Office hours Facility Information Facility accreditation 3

4 SELECT MED INTRODUCTION Provider Quality and Patient Satisfaction Ratings Quality scores and patient satisfaction ratings are available for some clinics. These ratings are based on the treatment the clinic delivers and results from an annual member satisfaction survey. Clinics must have 30 or more completed surveys for their results to appear. To take advantage of this feature, visit our provider search. Search for a family medicine, internal medicine, or pediatric provider and click on Satisfaction and Quality Ratings in the doctor s profile. If a ratings link does not appear, then data is not available for that provider. Notice of Availability Enrolling in a SelectHealth or SelectHealth Benefit Assurance plan does not guarantee services by a particular provider listed in this directory. If you wish to receive care from specific providers listed, you should contact those providers to be sure that they are accepting additional patients for SelectHealth. Member Advocates SM To contact Select Health Member Advocates, call (Salt Lake area) or weekdays from 7:00 a.m. to 8:00 p.m. Member Advocates can help you with the following services: Schedule an appointment for annual exams, immunizations, and checkups and help you choose a doctor who provides the type of care you need. Find a doctor who is accepting new patients. Assist in choosing a facility or doctor closest to you with the next availiable appointment. Help you understand and maximize your benefits. Get an appointment for urgent conditions when your Primary Care Provider is unavailable. 5 SELECT MED INTRODUCTION RURAL HEALTH CARE PROVIDER NOTICE Pursuant to Utah Code 31A and R , if a healthcare provider is a credentialed staff member (a healthcare provider with active staff privileges) at an Independent Hospital or a Federally Qualified Health Center not contracted with SelectHealth, you may still be entitled to coverage for healthcare services if you live or reside within 30 paved road miles of such healthcare provider, or if you live or reside in closer proximity to such healthcare provider than to a SelectHealth contracted provider listed elsewhere in this Provider Directory.
