1 NTBC CARNIVAL SPONSOR REGISTRATION FORM Dear Prospective Sponsor, I would like to extend my gratitude for all of you who have participated as sponsors for our church and school fund-raising event last year. These sponsorships are an invaluable contribution to spreading the gospel locally and abroad, training up children in the Word, and making disciples of men and women. This year we look forward to your cooperation to help sponsor our 8th Annual NTBC Carnival to be held February 21-24th. Becoming a sponsor will give you exposure to the thousands of families coming to the carnival and visiting our websites. Your contribution is tax deductible and going to a good and honorable cause for the support of our ministries. We will provide a donation letter for each contribution for your records. The following levels of sponsorship are available: CARNIVAL GRAND SPONSOR: $5, Opportunity to provide one company banner to be displayed one week prior to the Carnival Mention in Church bulletin for entire year (distributed weekly to both church locations) Company Name displayed on NTBC & DCS website homepage for one year (logo image linked to company site *If provided) Largest Company logo on the top of 500 Carnival t-shirts Included in all marketing materials for the Carnival Sponsor name mention during carnival over loud speaker throughout carnival days 4 Facebook posts Company Name and logo on color sponsor banner - 8' long with 12" letters to hang at center of the carnival. A 10 x 20 booth space with tables and chairs, with first choice of location in any lot (refer to attached Vendor Lot Map). Booth space for 4 days along with extra display space for activation or product. Opportunity to distribute collateral information or a gift to about 30,000 in attendance 4 complimentary Carnival Wrist Bands (unlimited carnival rides for all 4 days)

2 PLATINUM SPONSOR: $2, Mention in Church bulletin for 6 months (distributed weekly to both church locations) Company Name displayed on NTBC & DCS website sponsor page for one year (logo Company logo on 500 Carnival t-shirts Included in all marketing materials for the carnival Sponsor name mention during carnival over loud speaker throughout carnival days 2 Facebook post Company Name and logo on color sponsor banner - 8' long with 12" letters to hang at the top of the center stage A 10 x 10 booth space with tables and chairs, with choice of location from any Gold to Silver lots (refer to attached Vendor Lot Map). Booth space for 4 days along with extra display space for activation or product. Opportunity to distribute collateral information or a gift to about 30,000 in attendance. 4 complimentary Carnival Wrist Bands (unlimited carnival rides for all 4 days) GOLD SPONSOR: $1, Mention in Church bulletin (distributed weekly to both church locations) Company Name displayed on NTBC & DCS website sponsor page for one year (logo A 10 x 10 booth space with tables and chairs, with choice of location from any Gold to Silver lots (refer to attached Vendor Lot Map). SILVER SPONSOR: $ Mention in Church bulletin (distributed weekly to both church locations) Company Name displayed on NTBC & DCS website sponsor page for one year (logo A 10 x 10 booth space with tables and chairs, with choice of location from any Silver lots on a first come basis (refer to attached Vendor Lot Map [on pg. 4]). Booth space for 2 days (Thursday & Friday). Sponsors may include Saturday as one of the 2 allotted days for an additional $300, or Sunday for an additional $150. Opportunity to distribute collateral information or a gift to in attendance. Please note Silver Sponsor booths are limited and optional. We will give first preference to the Grand, Platinum & Gold sponsors. Silver lots are on a first come first serve basis.
