1 YOUTH COORDINATING FORUM (YCF)FOR THE PRAIRIES IS PLEASED TO PRESENT: REFLECTIONS RETREAT 2010 EXPRESSIONS OF ACOSMOPOLITAN ETHIC Dear Participant, We are very pleased to inform you that the Reflections Retreat 2010 will be held January 8-10, 2009 at Rundle Mountain Lodge in Canmore, right in the heart of the Canadian Rockies. The Youth Coordinating Forum (YCF) in conjunction with the Calgary Ismaili Students Association (CISA) strives to ensure that all youth between the ages of 18 and 25 receive the best quality religious education as well as social and artistic experiences in a diverse environment. The topic of this year s retreat is Expressions of a Cosmopolitan Ethic. This exciting and engaging topic will consist of four sessions and will be facilitated by Shahzeed Jiwa from B.C. and Zaheed Hasham from Edmonton. The cost of the trip includes transportation, accommodation, all meals, and supplies. Cheques should be made payable to The Ismaili Tariqah and Religious Education Board for the Prairies (ITREB Prairies). The cost per participant is as follows: Reflections Retreat 2010 Rates: Retreat Alone $ Retreat with Ski Lift Ticket $ Retreat with Ski Lift Ticket and Rentals $ Retreat with Ski Lift Ticket, Rentals and Lessons $ Retreat includes accommodation for 2 nights, all meals and transportation. For questions related to subsidies, please contact the undersigned. This package consists of a participant registration form, a waiver form and a packing list. Please return the registration form along with your signed waiver form and payment in order to confirm registration no later than Wednesday, December 31, Registration forms should be sent electronically to the address below. Payment will be collected in all Jamatkhanas beginning Friday, November 27, 2009, and at Post-Secondary Jamatkhana beginning Monday, November 30, Payment can also be mailed to the Ismaili Council for the Prairies, Attention: AKYSB, Suite #200, th Ave N.E., Calgary, Alberta, T2E 7A6. Please note that the buses will be departing from Headquarters Jamatkhana at 6:00 PM SHARP on Friday, January 8 th, & will be returning to Headquarters Jamatkhana on Sunday, January 10 th at 5:45 PM. In order to make this initiative a success, we would like to ensure full participation by all youth between the ages of 18 and 25, and hope that you will help us in achieving this goal. I further acknowledge that my son's/daughter's participation in the Reflections Retreat 2010, to be held in Canmore, Alberta on January 8, 9, & 10, 2010 and at Nakiska Ski Resort in Kananaskis on January 10, 2010 (the "Retreat") as organized by the Council for the Prairies (the "Organizer") has inherent risks and I freely accept and fully assume all such risks, and the possibility of personal injury and damage or loss to personal property resulting from participation in this activity. I agree as follows: 1. TO WAIVE ALL CLAIMS that I have or may have in the future against the Organizer, His Highness Prince Aga Khan Shia Imami Ismaili Council for Canada, and local Councils (including without limitation the Council for the Prairies) (collectively, the Council ), the Organizer, and other persons acting on behalf of the Council or the Organizer or assisting with the Retreat in any manner whatsoever (all of whom are hereinafter collectively referred to as the Releasees ), as a result of my participation in this activity due to any cause including negligence, breach of contract, or any other duty of care. 2. TO HOLD HARMLESS AND INDEMNIFY THE ORGANIZER AND RELEASEES from any and all liability for any damage to the property of, or personal injury to any third party, resulting from my participation in this activity; and 3. That this agreement shall be effective and binding upon myself, the participant namedabove, my heirs, next of kin, executors, administrators, assignees, and representatives in the event of my death or incapacity. Inentering into this agreement, I am not relying upon any oral or written representations or statements made by the Providers and Releasees other than what is set forth in this Agreement. Additionally, I understand that I have the opportunity to seek independent legal advice prior to entering into this agreement. I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE, WHICH, I, OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS AND ASSIGNEES, MAY HAVE AGAINST THE ORGANIZER AND RELEASEES.
