Due to national scandals, we understand that those who choose cremation might have doubt about the process and procedures of the crematory. Our 10-step cremation process guarantees that your loved one is well cared for at all times. This brochure will help answer any questions you may have about our 10-step cremation process, and the commitment we have to every family we serve.

3 One... Code of ethics At Swartz Funeral Home, we have a strict code of ethics and procedures we adhere to while your loved one is in our care. And, we only use Country Roads Crematory, a member of the Cremation Association of North America, who we know and trust to uphold these ethical standards. We take the extra steps to ensure that your loved one is well cared for at all times. Two... Safeguarding personal identity A personal identification tag is placed on your loved one the moment they are transferred into our care. Three... Unique tracking system Upon arrival at the crematory, your loved one is logged into a tracking system and assigned an identification number, which is stamped into a stainless steel identification tag. This number is used to track every phase of the cremation process. This tracking system includes date of arrival at the facility. Four... Family identification Our staff will properly encase your loved one in the cremation container selected; then, based on the information provided by the family, we will verify the identity of your loved one by comparing identification bands from the hospital or nursing home. In the case of a death that occurs at the family home, positive identification occurs when we arrive at the residence. Five... Checks and balances to ensure paperwork A crematory technician and licensed funeral director will review all cremation paperwork and permits. This is to ensure authorizations and permits have been received and have been properly documented. Once all documentation is verified, the technician will start the cremation process.

4 Six... Cremation log book Immediately prior to the cremation process, your loved one is taken to the crematory and their personal identification is noted in the cremation log book. This log shows the name of the individual, date of the cremation, identification number, and crematory operator s name. The crematory log is dated and initialed by a cremation technician before and after the cremation. Seven... Details during the cremation The crematory checklist and identification card are placed on the outside of the cremation chamber during the cremation process. The metal identification tag is present in the cremation chamber during the process. Eight... Details after the cremation After the cremation is complete, the cremated remains are placed with the stainless steel identification tag into an urn. The identification card and cremation certificate is placed into our permanent files. Nine... Return of the cremated remains Our staff will notify you to schedule a time for picking up the cremated remains. Ten... Celebration of life We will assist in planning a funeral or memorial service either before or after the cremation, which will appropriately celebrate the life of your loved one.
