4 INFORMATION, CODE OF CONDCT AND PARENTAL PERMISSION FORMS The parent(s) / guardian(s) of the youth participants must review the following information. If you wish your son / daughter to attend this retreat please complete, sign and return this permission form, the registration form and payment to Christ the King Church, 1599 Tunner Dr., Courtenay, B.C. V9N 8N3. Details: This youth retreat is for Vancouver Island Diocesan Youth & friends in Grades 8 to 12. It is being held at Christ the King Church, 1599 Tunner Driver, Courtenay B.C. The retreat starts on Friday, October 26 th, 2018 at 7:00 PM and ends on Sunday, October 28 th, 2018 at 12:00 noon. Transportation to and from the retreat will be the responsibility of the participants home parish. All meals, snacks, accommodation and field trip costs are covered in the registration fee. The participants will be overnighting in the two halls at Christ the King Church. There will be a field trip to a public swimming pool conservative, non revealing swim wear will be required. We will provide transportation to and from the swimming pool. At all times, the youth will be under the direct supervision of chaperones who have completed the necessary paperwork in keeping the diocesan Responsible Ministry Safe Environment (RMSE) policies. All retreat sessions must be attended by all participants. No participant can leave the retreat site unless accompanied by an RMSE chaperone. Participants who behave inappropriately or, are under the influence of or in possession of non-prescription drugs or alcohol; will be dismissed and transported home at the cost of the participant s family. There will be photographs and video taken during this retreat. These images may be used for parish and or diocesan promotions. Items to bring: Sleeping bag, foamie, pillow. 5 Code Of Conduct 1. No participant can leave the retreat area unless accompanied by an RMSE chaperone. 2. As ambassadors and leaders in our Christian community, all participants will show respect to other people and property at all times. 3. Participants will not consume or possess alcohol, non-prescription drugs or tobacco products. 4. Clothing that promotes, advertises or makes reference to the following is not permitted: a. Derogatory racial, religious or sexist messages. b. Vulgar, abusive and or inappropriate language. c. Criminal activity. d. Drugs, alcohol or tobacco products. 5. Clothing, including bathing suits, will be worn in a respectful, non-revealing way. Photography / Video Pictures and video will be taken during this three day event. If my son / daughter s picture and or video is taken, I give permission to Christ the King parish to use the pictures / videos for parish and diocesan reflections and promotions.
