2 Welcome to Faith Baptist Church Youth Ministry Faith Student Ministries Purpose Statement The purpose of Faith Student Ministries is to proclaim the truth and love of Jesus Christ to the world. Through regular meetings of fellowship and devotion we seek to serve and equip the students God entrusts us with in order that they accomplish this mission. We are committed to this cause and emphatically believe that God has called us together with them to serve Him now. Our hope is that through our commitment to God and each other we will develop relationships that will provide accountability and encouragement through the challenging years of adolescence. Core Values-Our core values reveal what is important to our ministry. In no particular order, these are the values we desire our ministry to reflect. Heart for missions Relationships over religion Musical worship Prayer Small groups Safe environment Christ Support for parents Bible based Evangelism 1. Prayer (Romans 12:12, 1 Thes. 5:17) Prayer and devotion is a vital aspect of our ministries atmosphere. Not just prayer that's flippantly thrown to the ceiling, but rather prayer that comes from a heart of devotion to God. 2. Unconditional Love (John 15:12-13, Romans 12:9-10) There are so many unconditional attitudes we seek to demonstrate through this: unconditional acceptance, caring, compassion, and a sense of security. It communicates to students that this is a place where they know they are genuinely loved without any prerequisites- they are welcome no matter what. 3. Respect (1 Peter 2:17) everyone has different opinions and perspectives. Although we have differences, we agree to respect each other and not allow it to affect our relationship. We do not push students to Christwe respect their wishes and let them discover Him at their own pace. 4. Encouragement (Hebrews 3:13) No one likes walking down a hard unknown path without support. Neither do students enjoy walking down an unfamiliar spiritual road without support and encouragement. Our ministry is build on motivation and support through encouragement in every area of a students life. 5. Hospitality (Romans 12:13, 1 Peter 4:9) We extend ourselves and our resources to each other. This includes material hospitality, but it also involves creating an environment that is comfortable and sensitive for unsaved students.

3 Faith Student Ministries Vision What we intend to produce Ministry has the capacity of accomplishing any number of outcomes. Counseling ministries hope to assist those with personal problems to recover and correct their situations in a way that glorifies God. Small group ministries desire to enhance spiritual growth by promoting Bible study through relational interaction and accountability. Worship teams hope to facilitate an environment where worship is promoted through music, thus guiding people s hearts into an attitude of reverence before the Lord. The student we wish to produce through the work of the Holy Spirit consists of specific qualities. First, the student will have left his or her former state of separation from God and has entered into a saving belief in Jesus Christ. This is followed by not only a desire to know God, but also the student will be motivated to leave their previous life of sin and change to life in worship, respect, reverence, and in awe of the Lord and eventually move to serving Him and others with their lives. Vision for Students Godly Genuine relationships are the key to our ministry. Everything else is supplemental to these relationships. When a student arrives at our youth group, he or she is meet with unconditional love and acceptance Various teams will be established for students to plug in to, with one or more adult mentors for each team. Teams include, but are not limited to: media, worship, outreach, teaching, and events. The students will connect with other area youth groups during the school year for the purpose of meeting and developing relationships with Christians their age. The youth group will prepare students so that when they graduate they are fully devoted followers of Christ and are ready to take their leadership skills and ministry experience into college and other parts of the world. 4 Faith Baptist Church Medical & Permission Form Please print in ink PARTICIPANT S NAME AGE BIRTH DATE ADDRESS HOME PHONE MALE FEMALE NAME OF SCHOOL GRADE PARENT(S)/GUARDIAN NAME(S) / WORK PHONE(S)/ CELL PHONE(S) / TO WHOM IT MAY CONCERN: The undersigned do(es) hereby give permission for our (my) child: ( Participant ), to attend and participate in Faith Baptist Church children or youth ministry activities, events, and retreats during the period of LIABILITY RELEASE: In consideration of Faith Baptist Church allowing the Participant to participate in children or youth ministry activities, we (I), the undersigned, do hereby release, forever discharge and agree to hold harmless Faith Baptist Church, its directors, employees, volunteers and agents (collectively herein the Church ) from any and all liability, claims or demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the Participant while involved in the children/youth activities. We (I) the parent(s) or legal guardian(s) of this Participant hereby grant our (my) permission for the Participant to participate fully in youth ministry activities, including trips away from the church premises. Furthermore, we (I) [and on behalf of our (my) minor Participant(s)] hereby assume all risk of accidental personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein. Further, authorization and permission is hereby given to said Church to furnish any necessary transportation (within the limitations of church insurance and the law), food and lodging for this Participant. The undersigned further hereby agree to hold harmless and indemnify said Church for any liability sustained by said Church as the result of the negligent, willful or intentional acts of said Participant, including expenses incurred attendant thereto. MEDICAL TREATMENT PERMISSION: We (I) authorize an adult, in whose care the minor has been entrusted, to consent to any emergency x-ray examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care, to be rendered to the minor under the general or special supervision and on the advice of any physician or dentist licensed under the provisions of the Medical Practice Act on the medical staff of a licensed hospital or emergency care facility. The undersigned shall be liable and agree(s) to pay all costs and expenses incurred in connection with such medical and dental services rendered to the aforementioned child or youth pursuant to this authorization. EARLY RETURN HOME POLICY: Should it be necessary for our (my) child or youth to return home due to medical reasons, disciplinary action or otherwise, the undersigned shall assume all transportation costs and responsibility. TRANSPORTATION PERMISSION: The undersigned does also hereby give permission for our (my) youth to ride in any vehicle driven by an approved ADULT chaperone while attending and participating in activities sponsored by Faith Baptist Church. 5 FSM code of conduct form 1) Respect and obey staff. Our staff care deeply about youth and wants to protect and nurture them. God has put us in positions of influence and authority for the purpose of teaching, correcting and training them in his ways. It is essential that they learn to submit to authority so that they will learn to obey God and have the blessings that come with obedience. Hebrews 13:17 "Obey your leaders and submit to their authority. They keep watch over you as men who must give an account. Obey them so that their work will be a joy, not a burden, for that would be of no advantage to you." 2) Stay with the group at all times. Students come to youth group to be with the group. There is rarely a need for a student or group of students to separate themselves from the group without staff permission. This causes disunity, cliques and hard feelings. It is also unsafe for the staff to be unaware of their activities or location. Ephesians 4:3 "Make every effort to keep the unity of the Spirit through the bond of peace." 3) Speak and act in love. Our youth group is a place where students come for encouragement, comfort, understanding and friendship. It is a place where we learn to work out our differences and difficulties with love and mercy. Behavior that is unloving will not be accepted. 4) Keep electronic devices TURNED OFF AND PUT AWAY (cell phones, games, ipods, etc). These devices distract youth from focusing on what is being taught and on building friendships with other students. It is selfish to ignore youth group members and staff by spending time using electronics while we are together as a group. This also includes the use of a electronic device to use as a Bible, we have plenty of Bibles available. (If a youth has a legitimate need to make a phone call, they must ask permission from a staff person. Parents can reach youth by calling a staff person.) If a student breaks this rule, a staff person will retain the device until a parent arranges to pick it up. 5) Dress with modesty. Our purpose in being together is to grow in Christ and Christian friendship. Therefore it isn't appropriate to dress in a way that causes others to be tempted to think about us in a way that is impure. If a staff person feels that your clothes are not modest, we will ask you to put on other clothes. For summer events, please wear a modest swimsuit and bring along t-shirt and shorts to wear during group activities. 2 Cor. 6:3 "We put no stumbling block in anyone's path, so that our ministry will not be discredited." 6) Refrain from public displays of affection Holding hands, sitting close, hugging, side hugging, kissing, back rubs, and all other forms of PDA will not be accepted because it is distracting and uncomfortable for the rest of the kids in the group. We encourage students to set a high standard of purity in their relationships, so that they can keep their focus on God 1 Tim 5:1-2 "Treat younger men as brothers, older women as mothers, and younger women as sisters, with absolute purity." 7) Foul language, drugs and weapons of any kind WILL NOT be tolerated. CONSEQUENCES FOR DISOBEDIENCE As Faith Baptist Youth Staff, it is our goal to handle problems immediately, consistently and in a spirit of love and gentleness. Using scripture, we will show students how their behavior is not pleasing to God and is harmful to themselves or the youth group. The staff will always seek a result that includes confession, forgiveness and restoration. Failure to follow the rules will result in the student experiencing one of the following consequences: Correction from youth staff. So that we do not humiliate a student, this will happen privately and will be carried out by 2 or more staff persons (for accountability). Whenever possible, one of the staff will be the same sex as the student. Dismissal from the event (a parent will be contacted for immediate pick-up) or suspension from a future youth group event. It is our desire for youth to be at every event, but when a student violates our rules, they interfere with other students' opportunity to learn about God and experience Christian community. It would not be fair or loving for us to allow a student to interfere with God's work in the lives of other students, so we will remove them from the group for a time if necessary. It is our prayer that they would use this time for self-reflection and return to the group with a right attitude and willingness to obey. Any time one of these measures is taken, the youth staff will notify the parent in person or by phone within 1 week of the offense. I have read the rules and consequences and agree to submit to them.
