Personal Spiritual Formation Assessment Spiritual formation is fundamental in the discipleship process. The only way we can be sure to grow up in our faith is to honestly reflect and evaluate ourselves. Through this assessment you will be able to track your growth in six key areas: giving - to build the kingdom and help others relationship - ministering to the church outreach - sharing the gospel with the world around us word - knowing, understanding, and living God s Word upward - worshiping the Creator corporately and personally prayer - spending time abiding in Christ As you complete the assessment, avoid rushing. Listen for God s voice to encourage and challenge you. Consider this experience as one on one time with Him. Be intentional in your growth toward being like Christ. Use the following scale to respond to each statement: never - 1 seldom - 2 occasionally - 3 frequently - 4 always - 5 giving - to build the kingdom and help others 1. I seek to give 10% of my income toward kingdom work 2. I try to give above my tithe toward special giving for projects or ministries 3. I try to give above my tithe for faith promise missions giving 4. I give my time not out of compulsion but out of careful & prayerful thought 5. I practice hospitality by sharing my home and possessions with others 6. I give time to help in the community 7. I give during the holidays to help the less fortunate 8. I give to the generous bucket at least twice a month to help the needy 9. I seek to live below my means to ensure I am able to give to meet needs 10. I view all of my earthly possessions as tools to advance the kingdom giving total
relationship - ministering to the church 1. I understand my spiritual gifts and often seek to use them to serve my church family 2. I serve others expecting nothing in return 3. I build relationships by regularly participating in G.R.O.U.P.S. of some kind 4. Meeting the needs of others gives me a sense of purpose in my life 5. I strive to share biblical truth with those I serve when God gives opportunity 6. I seek to find opportunities to combine my passions with my gifts to serve 7. I seek to admit my errors in relationships and humbly seek forgiveness when I have wronged someone, and confront in a loving way those who have hurt me. 8. I am slow to speak and become angry - and quick to listen 9. I help others identify their gifts and passions and help them become active 10. I am currently discipling or mentoring someone younger in the faith than me. (not in age but in spiritual maturity) relationship total outreach - sharing the gospel with the world around us 1. I pray for people who are far from Christ 2. I believe it is my job as a Christian to make disciples 3. I consistently look for opportunities to show God s love to others 4. I engage neighbors/co-workers/family/friends with the purpose of sharing God s love 5. I seek to live my life in such a way that my words and my life are my testimony 6. I am prepared to share my testimony at any time 7. I talk about my faith and look for opportunities to live it out 8. I support and participate in local, national, and international missions 9. I seek opportunities to participate in mission trips and outreach opportunities 10. I demonstrate a belief and commitment to the Great Commission, Matthew 28:18-20 outreach total
word - knowing, understanding, and living God s Word 1. I know where my Bible is 2. I regularly read and meditate on scripture 3-5 times a week (outside of church activities) 3. When scripture exposes and area of my life needing change, I respond by adjusting my life to reflect scripture 4. I use the Bible as the guide for the way I think and act 5. I regularly memorize scripture as a spiritual discipline 6. I have a regular routine and location for studying the Bible 7. I participate in some sort of Bible study with others 8. I seek further understanding by using other sources to help interpret the Bible. 9. I rely on scripture to help me share Christ with others 10. I evaluate cultural ideas and lifestyles by biblical standards word total upward - worshiping the Creator corporately and personally 1. I spend time daily worshipping God 2. I regularly attend worship services with the church family 3. I trust Christ to help me through any problem I might face 4. When God makes me aware of His will, I immediately follow it 5. I experience life change as a result of my worship experiences 6. I participate in worship gatherings outside of Sunday mornings 7. I regularly encourage others to attend worship services with me 8. I seek to worship God, even if the style of the service isn t what I like 9. I look for opportunities to spend time worshiping God in His creation 10. My daily life reflects an attitude of worship to God upward total
prayer - spending time abiding in Christ 1. I practice a regular quiet time that includes prayer 2. When making choices or decisions I seek Christ s guidance first 3. I remain confident of God s love and provision during difficult times 4. My prayers focus on discovering God s will more than expressing my needs 5. I pray daily, outside of meal times 6. I believe my prayers impact the lives of others 7. I pray expecting an answer, and listen even if the answer is not the one I want 8. I pray because I am aware of my complete dependance on God for everything in my life 9. I try to maintain an attitude of prayer all day 10. I offer to pray immediately with a person rather than just saying I will pray for them prayer total Now enter your totals by shading up the column according to the number you had...in other words if you score a 35 in giving, you would shade 3.5 blocks up from the bottom of the graph. By filling out this graph, you will have a grasp of which areas you are strong, and those in which you are weak. Just because you score low in an area does not mean that you can t do it, it just means you need to develop that area. Each area should be an area that we are striving to be better in as we grow. points giving relationship outreach word upward prayer
Now that you have charted your scores, take a moment to pray for the areas where you have scored low. Use the questions below to spark your next steps toward spiritual growth. 1. What three things do you need to do to help you grow in each of the six areas of growth? Write the three things down under each of the headings. giving - relationship - outreach - word - upward - prayer - 2. What actions do you need to take to remove the barriers to growth in the six areas of growth? giving - relationship - outreach - word - upward - prayer - 3. What is the time frame for starting your plans and action steps? 4. Who can you share these plans with that will hold you accountable?
