Every teen must participate in at least one of the St. Ursula Church sponsored service opportunities listed above. Please note that Saint Ursula School events will NOT count as a St. Ursula Church sponsored opportunity (i.e. Harvest Festival, Trunk or Treat, etc) but can be counted towards the remaining required hours. After completing the mandatory service, turn in this log with the reflective essay explaining your service opportunity. If you need more space, attach additional sheets. Date Service Activity Number of Hours Supervisor Signature In one page, respond to the following questions about your most memorable service opportunity: 1. Describe the work you did at your service opportunity. 2. Describe the organization in which you worked. What is the mission? What targeted group is this organization trying to reach? 3. How did this experience relate to your Confirmation preparation? 4. In what ways did this experience connect to your faith? How did this experience help you to encounter God in our world?

5 St. Ursula Parish Confirmation Letter to the Bishop As part of all Confirmation programs in the Archdiocese, each candidate is asked to write a letter to our confirming Bishop stating your desire to be confirmed and also why it is that you are making this choice. Often, the Bishop will use pieces of these letters in his homily at your confirmation. It is important that you keep this letter to one page, but answer the questions the Bishop asks thoughtfully. Your letters should be typed. YOU MUST BRING YOUR LETTER TO THE CONFIRMATION SESSION ON SUNDAY, MARCH 18th. We will then send all the letters to the Bishop s office in one envelope. Some guidelines for writing a letter: 1. Letters to the Bishop are formal, and that means they should be typed. 2. Include a greeting such as, Dear Bishop Madden, or Dear Archbishop Lori, 3. In your first paragraph be sure to introduce yourself and request that you would like to be confirmed. Introduce yourself with a sentence about you and your family. Give some reasons why being confirmed is important to you. 4. In a following paragraph, explain how you have prepared for the sacrament. Talk about things like your prayer life, confirmation classes, service, your retreat experience, the things you have done within the parish, etc. 5. Tell the Bishop what Confirmation name you have chosen and why you chose it. 6. Make sure there is a sentence or two that concludes your letter by thanking the Bishop for reading your letter and for confirming you and your peers. 7. End with, Sincerely, and sign your name. LETTERS ARE DUE IN CLASS ON MARCH 18, 2018.

6 St. Ursula Parish Confirmation Letter to Sponsor Dear Sponsor, You have been chosen to be a Confirmation sponsor, congratulations! This young person sees in you a profound faith commitment, finds trust and knows you are someone who will care about them and their faith. Sounds impressive, doesn't it? If you're a practicing, mature Catholic, you needn t worry. It's not all that complicated, but it does take time, a caring heart and a listening ear. First, please be sure to submit the enclosed SPONSOR INFORMATION FORM as soon as possible. We also suggest that you meet periodically with your young person in prayer and reflection as they continue to affirm their desire to receive this sacrament. While the way in which you choose to mentor your young person depends upon your relationship with them, we hope that you are able to make time for them during this sacrament prep and for the years that follow. Here are a few suggestions on how to be the best possible sponsor for your candidate: Pray for your candidate and yourself. As candidates decide to be confirmed, they need spiritual strength. Your prayers for them are important, but don't forget to pray for yourself also, that you can share why you value and practice your Catholicism: attend Mass together or even have the courage to come to Reconciliation together. Give of your time and share your gifts. Spend time with your candidate! Take them to lunch, go out of your way to engage them in conversation about their Confirmation process, their own faith life, and the things they are questioning and pondering! Let your unique God-given talents and gifts shine! Offer a gift of spiritual significance a new Bible, rosary or book about our faith or about the saint the candidate has chosen for a Confirmation name but be creative and relevant! Don't miss the ceremony or the rehearsal! Participating in the ceremony is the easiest part of being a sponsor. Your basic job will be to place your hand on your candidate's shoulder, and tell the bishop your candidate's Confirmation name. You're there to be a support, but your role on Confirmation day is only beginning. REHERSAL WILL BE ON TUESDAY, MAY 1ST AT 7PM. We will meet in the Spiritual Center to line up and then continue upstairs to the main church. CONFIRMATION MASS WILL BE CELEBRATED IN MID-MAY, DATE TBD BY BISHOP S OFFICE. Don't forget this newly confirmed Catholic. After Confirmation day, remember birthdays and this anniversary. Send a card or make a phone call. Continue to worship together, or from time to time do some Christian service. Put those "Gifts of the Spirit" into practice. God bless you and your candidate as you embark on this faith journey together! Sincerely, The St. Ursula Confirmation Team
