2 Dear Parents, The purpose of the Discipleship Award Program is to help the young people of St. Lawrence learn, explore, and experience our Catholic faith in a way that leads them to a greater understanding of and a deep, lifelong commitment to our faith. It is important for our children to learn that we are not passive spectators in our relationship with God. Each and every day, we must engage ourselves in activities that draw us closer to God and to our final goal of eternity with him in Heaven. It is the goal of this program to give our children opportunities to do just this, engage in activities that draw them closer to God, and to recognize them for their efforts. The Discipleship Award Program is an optional program for those students and families who wish to use it. There is an award booklet for each grade level, Kindergarten through 8th Grade. Each grade level booklet will contain age appropriate activities. It will be important however, especially with younger children, for parents to be actively involved in assisting children with completing the program requirements. In fact, some of the activities included are family activities. The Church teaches that Parents are the principal and first educators of their children (CCC 1653), and children will receive the most benefit from this program if families are actively involved in the learning process. How the program works: Each year, students will have the opportunity to achieve the discipleship award three times (see due dates to turn in paperwork on the following pages). If a student earns all three awards in a given year, he/she will receive special recognition in the form of an additional award, called the Trinity Award. Students who achieve the Trinity Award for 3 or more consecutive years are invited to an annual dinner in their honor. The activities within the program are separated into four categories based on the Four Pillars of the Catholic Church: 1) What We Believe; 2) How We Worship; 3) How We Live; and 4) How We Pray. Some activities within each category will be required, and some of the activities will be listed as options within each category. The due dates and directions for each of the award periods are given on the following pages. Homeschoolers: When instructions are given to share something with a class, the student can share the project at a homeschooling or similar type gathering. We look forward to partnering with you in encouraging our students to grow in knowledge and faith. 3 First Grade Discipleship Award Part A Due Monday, November 5th The requirements for this award are listed below. This form and any required attachments must be turned in to the Campus Ministry office by Monday, November 5th. (Form can be turned in to the school or Faith Formation office, or mailed to the Campus Ministry Office 5223 N. Himes Ave. Tampa, FL ) Section One: What We Believe REQUIRED ACTIVITY We believe that God is our Father who created the whole world and that everything he created is good. With your family, read from the Bible: Genesis 1:1-31. Draw a picture of what the first chapter of Genesis teaches us about creation. Attach the picture to this page. CHOOSE ONE OF THE FOLLOWING ACTIVITIES Option #1: With your parents, go online to the following site: Click on the icon for We Believe Catholic Identity Edition Grade 1. Click on the small blue arrow on the top of the page to take you to the next page. Scroll down to the Games and Study Aides section. Choose the game named Matching Activity-God s Gifts. Play the game at least two times to see some of the gifts God gives us. Write down what picture you uncovered each time. Option #2: We believe that the saints are examples of holiness for us. With your parent s help, go to the following site: Click on the icon for The Lives of the Saints. Choose one of the following saints: St. Thomas More (June) or St. Peter the Apostle (June). Click on and print out the Primary Activity. Complete the activity and attach it to this form. Section Two: How We Worship REQUIRED ACTIVITY As Catholics, we believe that the Sunday celebration of the Lord s Day and his Eucharist is at the heart of the Church s life (CCC 2177). It is important that we gather as a Church family each Sunday and on Holy Days to worship. When you attend Mass on Sunday or a Holy Day, USE THE I attended Mass today STAMP AT ONE OF THE CHURCH EXITS TO STAMP THE APPROPRIATE DATE on at least 7 of the following weekends/holy Days. Don t forget to say hello to the priest or deacon after Mass. Sept. 1/2 Sept. 8/9 Sept. 15/16 Sept. 22/23 Sept. 29/30 Oct. 6/7 Oct. 13/14 Oct. 20/21 Oct. 27/28 Nov. 1(All Saints) Nov. 3/4 3

4 CHOOSE TWO OF THE FOLLOWING ACTIVITIES: Option #1: Take up the gifts at a Sunday Mass with your family. Parents should call Mrs. Boston ( ) to schedule this. Write the date you took up the gifts here. Option #2: Dress up as your favorite saint and participate in the Saint Procession at the 9:30 Mass on Sunday, October 28th. Saints should arrive no later than 9:20 and report to Higgins Hall. Use the I completed this activity stamp at the back of the church to stamp here: Option #3: With your family, write the names of your deceased family members in the Book of the Deceased which is displayed in the church during the month of November, OR place photos of deceased loved ones on the Wall of Memories which is displayed in the church during the month of November. Write the date you wrote the names in the book or hung the pictures here. Option #4: With your family, attend the All Souls Procession and Mass on November 2nd at 7:00 PM. See the bulletin or website for details. Have the priest or deacon sign here. Option #5: Draw a picture showing your favorite part of the Mass. Ask your teacher for permission to share your picture with your class. After you have shared your picture, have your teacher sign here: Option #6: With your family attend the Family Faith Fest celebration of the Diocese 50th Anniversary, October 27th at Al Lopez Park. Stop at the St. Lawrence table sign in. Take a picture of you and your family at your favorite event and attach it to this packet. Section Three: How We Live We believe that God wants us to care for others, especially those who are poor, sick, and lonely. CHOOSE TWO OF THE FOLLOWING ACTIVITIES Option #1: With your family, make a card or gift for somebody who is sick or homebound. Visit that person to deliver the card or gift. Write the name of the person you visited here. Optional: Take a picture with the person and attach it to this form. Option #2: Shop as a family for food to donate to the Matthew 25 Food Drive on one of the following Wednesdays after school: Sept. 19 or Oct. 24. Write the date you donated food here: Option #3: Select one of your toys to donate to charity. As a family drop the toy off to the charity of your choice. Write the name of the toy and charity here. Option #4: Donate 10% of your Halloween Candy to the Annual Candy Collection (Nov. 1-5). Write down the date you donated the candy here. Option #5: With your parent s help, choose one way to help a member of your family or a neighbor. Take or draw a picture of you helping this person. Attach the picture to this form. 4

5 Section Four: How We Pray CHOOSE TWO OF THE FOLLOWING ACTIVITIES: Option #1: Take your pet to the Blessing of Pets and Animals on September 30th at 3:00 PM or October 4th at 7:00 PM. Have the priest or deacon sign here. Option #2: During the month of October, visit the Marian Shrine in the church. Pray a Hail Mary at the shrine. Write the date you visited the shrine here: Option #3: Learn the Morning Offering and Evening Prayer and recite these for your teacher, Mrs. Roman, or Mrs. Williford. (These prayers can be found in your religion book or by using the site mentioned above in Section One. Just page down to Prayers.) Have your teacher, Mrs. Roman, or Mrs. Williford sign here. Option #4: Make up your own prayer dedicated to Mary and write it down. Your parents can help you if you need them to. Draw a picture to go with your prayer. Attach the prayer and picture to this form. Option #5: With your family, make a special visit to the church on a weekday to spend some time with Jesus and to pray for all of your friends and family. Write the date you visited the church here. Option #6: With your family, participate in the parish Traveling Chalice Program to pray for vocations. Contact Mrs. Boston ( ) to schedule your family s week with the chalice. Once you have the chalice, follow the instructions provided and pray each day of the week, as a family, for vocations. Take a picture of your family with the chalice and attach the picture to this form. 5

6 Award Completion and Submission Form When you have completed all the requirements for Part A of the Discipleship Award, complete this form and submit it with the Award form and any other required attachments. All forms and attachments are due by Monday, November 5th.
