Statement of Faith The following is the statement of faith of Covenant Christian School (CCS). The curriculum teaches these truths. Please read the statement carefully. For your student to have a successful experience at CCS, we feel that parents must be in agreement with the spiritual principles taught at CCS. Parent Agreements, Consents, and Disclosures Parent Support Agreement-I/we recognize that Covenant Christian School (CCS) has a qualified, trained staff, and I/we have confidence in their ability to perform the educational functions due us at their discretion. I/we realize that from time to time students take issue with actions that they do not agree with and are prone to criticize statements out of context. This being normal for children, I/we pledge that should such occur, I/we will not support the criticism. I/we will correct our child, support the school personnel, and call for full details at any time I/we have a question concerning the incident. The training of our child is as much our responsibility as it is the schools. I/we will pray for the staff and the school; cooperate with them in discipline; lay a spiritual foundation through Godly examples in the home; follow through with any work, assignments, or slips to be signed; see that the child reaches school on time; call the office or send written excuses for absence or tardiness (as outlined in the Parent-Student Guide); cooperate in training the child to respect school property and pay for irregular abuse of the same; attend parent functions; and assist in positively publicizing the school and its program among friends. I/we understand that enrollment at CCS is not a right, but a privilege, and do affirm that I/we believe that discipline is necessary for the welfare of each student. If this application is accepted, I/we give permission for our student s teacher and/or other agents of the school to make and enforce classroom regulations in a manner consistent with Christian principles of discipline as set forth in Scripture.. If we ever find that we cannot accept the disciplinary standards of CCS, we will withdraw our student from the school. In that event, or if for disciplinary reasons the school must release our child, I/we will forfeit all fees and tuition money paid. Book Agreement-Students will be issued textbooks during the school year. Students will return these books, with the exception of workbooks, to the school at the end of the school year in the same condition in which they were received. If a book is lost or damaged it is understood that the student is responsible for the cost of replacement or repair. Emergency Medical Treatment-In case of emergency or accident, I/we by signing this form give consent for first aid/and or medical treatment for the student named on the cover page of this application. I/we will accept responsibility for payment for such treatment unless all of the charges are covered by insurance. Transportation and Field Trip Consent-I/we give consent for our child to attend field trips sponsored by CCS. It is understood that transportation will be by bus, private automobile driven by a responsible adult, by train, volunteer or parent, or by walking to nearby locations accompanied by a responsible adult approved by the school. Further, I/we agree that CCS, its staff, or any approved person selected by CCS to accompany the students will not be held liable for any injury or accident that may occur in connection with any field trip. Every CCS student is covered by a school accident insurance policy. Combined/Split Classes-When determining yearly enrollment, limited classroom space or a fluctuating number of students at a grade level may necessitate the combination/splitting of two grades or classes. Parents will be informed when and if this determination is made. Tuition fees will remain the same regardless of the class combination or split. Picture Use Permission-I give CCS permission to use my child s picture for the CCS website and for promotional use. 6 V O L U N T E E R P R E F E R E N C E While Covenant Christian School tries to keep the tuition and fees down to provide an affordable private Christian education, we do have many needs. All of the many volunteers that give countless hours of behalf of the school are really working for God s kingdom by helping keep our costs down which enables us to offer lower tuition and fees than other schools. We encourage all of our families to get involved and be part of what God is doing in and among us. When we freely give and share our time and talents, it is a great example to our children of the Body of Christ in action! Please indicate your volunteer preference and let us know some idea of days and times that you are available.
