The attached information explains the process to use in requesting a portion of the CASA funds for particular needs. To assist you in applying for these funds, guidelines for grant requests are enclosed. Please read the guidelines carefully and follow them in writing your request. The following must be included with your submission: Grant Application Form Signature of Pastor, Parish Catechetical Leader, Youth Minister or designated person filling out application All other pertinent information needed from the guidelines Please note the request must be received by 5:00pm, Monday, September 18, We regret that we will not be able to consider those applications that do not comply with the deadline or procedures, so please get your application in early! 2 OFFICES OF FAITH FORMATION AND YOUTH MINISTRY CASA GRANT GUIDELINES I. PURPOSE OF GRANT CASA Grants are intended to financially assist Faith Formation and Youth Ministry Programs in providing direct services to children, youth, and adults. II. FUNDING PRIORITIES Parishes that do not have access to adequate sources of funding for: textbooks, materials and resources that would enhance the quality of their catechetical and youth ministry programs. training/continuing education of parish catechetical leaders, youth ministers and volunteers. developing and upgrading their technological resources. III. POLICIES. Requests must be made on the application provided by the Offices of Faith Formation & Youth Ministry and must be submitted by the pastor, parish catechetical leader, or youth minister. All elements of the application form must be completed in order to be considered including the signature of the parish pastor. A. A parish may submit more than one request. Separate grant applications are to be submitted for each sub-department (ex: Religious Education, RCIA, Youth Ministry, Adult Formation, etc). B. Grants can be awarded up to $3,000 total per parish for all ministerial departments combined. Awards for grants will be paid directly to the parish submitting the application with an explanation of how the money is to be utilized for each department. If awarded a grant this year, the parish will have until December 31, 2018 to use these funds and will need to submit receipts verifying purchases for audit purposes and to complete the application process. C. Usually, requests for 100% funding are not considered. Greater consideration will be given to requests which demonstrate some funding from sources other than the CASA Grant alone. Completed application forms must be received by 5:00pm, Monday, September 18, 2017 at the Diocesan Office of Faith Formation. A. Describe the textbooks/resources, training, and technology for which the grant is desired. You may attach any orders or documentation that will support your request. a. Textbooks and/or Resources (include the following elements) i. Name of publishing company and/or specify the type(s) of resources needed (DVD s, CD s, Bibles, etc) ii. Quantity of items requested b. Training (include the following elements in your explanation) i. Name and location of the training and/or continuing education ii. Name(s) of participants attending iii. Indicate their title/position, years of service in respective ministry and whether they are volunteers or parish staff c. Technology Equipment (include the following elements) i. Specify the type of technology equipment (DVD players, CD players, TVs, etc) ii. Provide specific details about the equipment and quantity needed B. Please explain how these materials and/or training will enhance the quality of the programs that you provide. C. How will the purchase of these materials and/or training assist you to meet your ministerial goals, specifically in the area of catechesis? D. Will these materials be used in collaboration with other departments within your parish? If so, please specify the various departments. E. Please describe how the parish plans to fund these efforts beyond this year.
