1 Liability Form I circle one: Advisor, Priest, Chaperone) assume all responsibility for every member from my parish attending the MIDWEST SOYO Retreat/Basketball Tournament, hosted by St. George Antiochian Orthodox Church, Canton, Ohio. I agree that the Archdiocese and St. George, Canton, are not, under any circumstances, responsible for any actions including, but not limited to, personal injury, vandalism, desecration of property, or theft. I agree that under the Archdiocesan Code of Conduct underage consumption of alcohol, tobacco and illegal drugs is strictly forbidden, and that in the spirit of Christian virtue, fellowship and athletic competition even those of legal age will not drink in the presence of minors. I agree that all dress will be modest and clean, that shorts, tightly fitting clothing, tube or tank tops, short skirts/ dresses, low cut tops/dresses, and that T-shirts with logos, writing, etc. are not appropriate attire for the Divine Services or any other Church event, including the Hafli and teen dances. I also agree to uphold and be subject to all rules regulating chaperoning and curfews. Please have each attending participant of your parish sign their name legibly on the lines below, confirming that they will abide by the aforementioned Code of Conduct, understanding that anyone found breaking the Code of Conduct may be asked to leave the hotel, the tournament or the Hafli, and that any adult found providing alcohol to minors will be dealt with to the full extent of the law. Please remember that this is a Church function and we must all represent our Church with dutiful Orthodox Christian behavior!
