1 Eucharistic Minister Application Expectations and Guidelines for Eucharistic Ministers: 1. Read the application packet in its entirety before beginning. 2. By May 16, 2011, submit the application and one letter of recommendation (form located within the application). One letter should be from one of your parents or one of your parish ministers, i.e.: pastor, associate pastor, youth minister, director of religious education/or the director of sacramental preparation, etc. 3. You need one faculty referral. The forms are included in this packet. Ask each faculty member to return the form directly to Ms. Danysh s faculty mail box before May 16 th. 4. Complete the application packet and give to Ms. Danysh before the end of the school day on May 16 th. Make sure your own statement as well as your parent/parish letter is included. No phone calls will be accepted for the parent/parish letter. 5. Please note there will be a half-day training session afterschool on the first day of school (Friday) 6. By choosing to be a Eucharistic Minister, you are choosing to make community and spiritual development a committed priority. This requires a clearly responsible commitment of time. School schedules, activities, jobs, car-pools, and outside commitments/responsibilities need to be taken into consideration before taking on any additional commitment. 7. Eucharistic Ministers are asked to (when available) serve as a Eucharistic Ministers for additional masses like weekday masses/retreat masses once a month at daily morning mass in the chapel. 8. Eucharistic Ministers are expected to worship with their own parish communities each Sunday/weekend. 9. Eucharistic Ministers are expected to model the religious goals and ideals of Jesuit College Prep to the best of their ability throughout the year.

3 Questions: 1. Describe your relationship with Jesus Christ, the Catholic Church, and the faith community at Jesuit. 2. Why do you want to be a Eucharistic Minister? 3. By choosing to be a Eucharistic Minister, you are choosing to make community development a priority. You are expected to participate fully in monthly liturgies as well as at least one supplemental liturgy throughout the year (class retreat masses, etc.) You are also expected to meet once a month for morning liturgy with your fellow Eucharistic Ministers. Please comment on your ability to fulfill these commitments throughout the school year. 4. Eucharistic Ministers must be baptized Roman Catholics and are expected be already confirmed.
