1 Procedures for Extraordinary Ministers of Holy Communion as Regards Homebound and Hospital Visits on Behalf of Our Lady Help of Christians Parish, Abington, PA May, Extraordinary Ministers are always to present themselves dressed appropriately for their task, in other words as one would to attend Mass. 2. Abington Hospital visitations require registration and training as a hospital volunteer. Official volunteer ID badges must be worn. Contact the AMH Volunteer office for details. Please call or check out 3. Homebound and hospital visitations require adequate time to reverently present the Blessed Sacrament to the patient, but also to separately listen to the homebound (and to a lesser extent to hospital patients) and respond to their questions and concerns in a friendly, relaxed manner. The time required will vary from patient to patient, but the important point is not to rush the process. Be on time. It is often best to call homebound patients prior to visiting. Outside of transporting the Blessed Sacrament, patients need to be listened to attentively. Some may want to talk a lot, others very little. They may also appreciate your talking as well, but listening is second only to Jesus. 4. Hospital patients may be visited unannounced. Lists are only available after 9AM. 5. Homebound and patients in a hospital are exempt from canonical fasting requirements that normally apply to healthy people. 6. Never retrieve the Blessed Sacrament the night before homebound or hospital visits. Special permission of the bishop (and pastor) are required to keep the Blessed Sacrament anywhere outside of a church. Always retrieve the Blessed Sacrament just before visiting the hospital or homebound. 7. Extraordinary Ministers are authorized only to bring the Blessed Sacrament to members of Our Lady Help of Christians Parish or Abington Hospital patients, as authorized by the Pastor. Special permission of both the OLHC Pastor and the Pastor of the other Catholic Church are required in order for an EM to travel outside of the Parish. Generally, this is to be avoided, unless one is ordained as an Acolyte, Deacon or Priest. 8. Homebound and hospital patients appreciate being given a copy of the Parish Bulletin. Be sure to take copies with you to give to them as you are leaving the hospital room or home. 9. When retrieving the Blessed Sacrament, proceed to the Sacristy where you will find a pyx in the center top drawer of the counter closest to the altar. Never transport the Blessed Sacrament any other way than in a blessed pyx. Some EMs have their own private pyx, which is fine as long as it has been blessed by a priest and it has been properly purified after each use. 10. Do not interrupt Mass. If Mass is not going on, you may proceed to the tabernacle, open the door, remove a ciborium and take off the lid. Then you should genuflect, if you are able. If you are retrieving the Blessed Sacrament by yourself, you may transfer the appropriate number of hosts to your pyx. If there is a group, take the ciborium to the altar, where each EM may retrieve hosts one-by-one. The last EM should take the ciborium back to the tabernacle, replace the lid, repose the ciborium in the tabernacle and close and lock the door. Do not remove the key. There is no need to genuflect now, since you have the Blessed Sacrament with you. Do not repose an empty ciborium. Take it to the counter in the sacristy and leave it for the priest/deacon to purify. 1
2 11. A pyx may be carried in a pocket or purse. The most reverent way to carry a pyx is in a burse, worn on a string around the neck. When entering the home or hospital room it should be taken out and displayed unopened until time for Communion. 12. The rite of Communion in a Home or Hospital is attached and should be used as prescribed. Patients often appreciate hearing one or more readings from the day s liturgy, so be sure to bring a missalette with you and read those readings aloud if circumstances permit (almost always in the home, but the hospital may be more time constrained). If your feel comfortable you may also read the day s Gospel in a home or hospital setting (only ordained ministers may do so at Mass). Since the Gospel is central to God s word, sick patients deserve to be able to hear it proclaimed. 13. Be sure to keep a supply of notes for hospital patients in your prayer book. Packets of these notes are in the pyx drawer and are to be used when you visit a hospital room and the patient is not available. Leave a note and a bulletin there in this case, noting when you visited and why you were not able to give communion. These reasons include that the patient was asleep, gone, or with a doctor/nurse. If it is practical to wait for the patient, do so, but you are under no obligation. Likewise if it is practical to return later please do so. 14. We are volunteers at AMH, with no medical authority whatever. Hospital personnel always rule. We meekly follow their directives regarding contact with patients. We do not interrupt anything resembling medical treatment or advice. We do not generally wake up patients unless they have previously given a specific request to receive Holy Communion. As always, use your sensible judgment and ask a responsible AMH employee if you have any questions. 15. When visiting the hospital you may use the parking spaces in front of the Elkins Building labeled Clergy, but only on Sunday mornings while bringing communion. In this case, visibly place a church bulletin inside your windshield. If these spaces are filled, use your parking pass and assigned garage. It is best if you leave your hat, coat and gloves in your car if possible. We no longer have access to any space for this inside AMH. Your alternative is to leave them behind the receptionist if he/she allows that, but do not wear them to patient rooms. 16. Proceed directly to the Widener Building (Shorday Lobby) and sign in electronically with your hospital ID number at the kiosk near the elevators. Retrieve the patient lists for your assigned floors at the reception desk. They are in a white binder. Visually scan the lists and circle the room numbers with Catholic in the religion column. 17. At the moment, we only visit the following floors: 2W, 3W, 4W (Widener Building); 3H, 5H, 6H (Buerger Building, formerly called Highland Building); and 2L, 5L (Lenfest Pavilion). We do not at present visit the Toll building, the Psych ward or Maternity, but I expect some of these floors to be added at some point. 18. Go to the nurse s stations on each assigned floor first. There are two on each floor. Ask the attending nurse which of the names you have circled are safe to receive Holy Communion. Visit only those patients whom the nurse authorizes. 19. Use the waterless hand cleaners found everywhere in AMH to Gel-in, gel-out of each room. This protects both you and the patients from cross contamination. Remember, for purposes of AMH regulations, EMs are Staff and subject to all posted rules for staff members, even though we have no rights or privileges. We are not visitors. 20. Knock on entering each room and announce your presence by confirming the patient s name and stating Hello, I am John Doe from Our Lady Help of Christians Roman Catholic Church. Would you like to receive Holy Communion today? Please say this just as it is written, inserting you own name. It is important that the patient recognize that you are Roman Catholic and that he/she understand the meaning of receiving Holy 2
3 Communion. Occasionally we are directed to non Catholic patients. They may not receive Communion. It is also possible that the patient is unable to physically receive Communion (such as Nothing By Mouth formerly Non Pro Oro or NPO) or some other physical reason such as a tracheotomy or breathing/feeding tubes. Generally we do not bother patients like these, although NPO patients may frequently receive Holy Communion if they appear able, they want to, and the floor nurse authorizes it. Use your judgment; nurses usually find Holy Communion to be okay even for NPO patients, but the hospital personnel always have the final decision even if the patient wants it. If for any reason the patient may not receive communion, either for physical reasons or another reason such as them saying they have not been to Confession, you should use your judgment to determine if you should offer to say some prayers with them. This is not always appropriate. Most patients want communion. Those who do not for spiritual reasons (like Confession) will usually say why they do not. In this case, it may be appropriate to offer to pray with them. If they simply say No, then thank them and leave. If they agree to pray, offer to lead some simple prayers such as the Our Father, Hail Mary, Glory Be and perhaps the prayer to St. Michael on the back of the Missalette. 21. Note that visitors in patient rooms may also receive Holy Communion if they are Catholic and want to do so. Canon law prescribes that Catholics may receive Holy Communion two times in a day, but the second one must be at Holy Mass. Technically, one can receive three times, but the third one must be Holy Viaticum for the dying. You should realize that you are dealing with sick patients and when you give them Holy Communion, it may very well be Holy Viaticum ( Food for the Journey ) for them. 22. Another case where your judgment is crucial is when there are signs on the area around the door to hospital rooms. Nothing By Mouth is one of these. Contact Precautions or Infectious disease precautions is a red flag for your own safety as well as that of the patient. Frequently, visitors are required to don one of those yellow gowns from a drawer on a cart outside the room and use non-latex gloves. You should decide if you want to visit such patients. I decide on a case by case basis. I do not always visit such rooms, but occasionally I do if I feel safe in doing so. There is no reason for you to risk your own health or anybody else s by doing so. You are morally justified in whatever decision you make; just let the Holy Spirit guide you. If you choose to visit, introduce yourself first as always. If the patient wants communion you will have to wear the gown and/or non-latex gloves. When done, put the gown in the bag on the cart outside the room and throw out the gloves. Use a separate set at each room you visit. It seems that CP/IDP signs are getting much more common. 23. Swallowing Precautions is another sign you must look for and obey. Usually such patients can receive a small piece of the host, but you must clear this with the nurse. 24. Whether or not there is a sign, patients sometimes cannot swallow a whole host. They receive the entire Body, Blood, Soul and Divinity of Christ from even the smallest particle of a host. In these cases, break off as small a piece as you and the patient can handle and present it. It may be dropped into the open mouth if necessary. Feel free to offer water immediately after Communion if it appears needed. Be careful to look for a water cup on their tray first, since some patients are not allowed thin liquids No Thin Liquids sign is posted). Such patients should only be given a very small part of a host. 25. Sometimes a patient actually expels the Blessed Sacrament for unpredictable reasons. Do not panic. Jesus understands. Retrieve what you can in a tissue and bring it back to the secrarum where you may let it dissolve in the flowing tap. Do the same if a host ever drops to the floor in church or elsewhere. Do not give such a host to another communicant. 3
4 26. Patients will often ask if they may make their Confession or receive the Sacrament of the Sick. Sr, Michael coordinates visits by Priests during the week for this purpose. Ask them to call Extension 8090 on Monday to Friday to ask for a visit. 27. Please make liberal use of the waterless hand cleaners found everywhere in the hospital. Use them at least before and after each room. 28. The Psych ward is a special case. It has recently been moved from 3B to 4H and at the moment it is not an area we visit but I expect that to change. I enjoy visiting that ward because most of the patients are being treated for depression and really appreciate Holy Communion. Generally you should not go directly to rooms in this ward, for your own safety. Always proceed to the nurse s station where you give a nurse the patient list and ask that those you have circled be invited to the common room for Holy Communion. When those who want to receive are assembled, use the same rite as you would for individuals in a communal setting. When all patients have received, you will need to be buzzed out of the ward by a nurse. 29. When finished with all of your assigned rooms in the hospital, be sure to return the list to the white binder with the receptionist and sign out electronically at the kiosk. 30. Always return to the Sacristy immediately after your assignment if there are any remaining hosts in your pyx. If Mass is not going on, proceed directly to the tabernacle, open it and remove a ciborium just outside the tabernacle. Remove the lid and place the remaining hosts inside. Replace the lid and return the ciborium inside the tabernacle and close and lock the door, leaving the key in the lock. You may purify your hands using the holy water next to the tabernacle if necessary. Then genuflect and return to the sacristy. 31. If Mass is going on, you may place the closed pyx on a new corporal from the drawer in the center of the counter nearest the altar. If you are assigned to Communion at Mass, and see pyx(s) there, please bring them to the Altar so the priest may combine them in a ciborium before Communion. If you return to church after the tabernacle has been locked, you may leave the pyx on the ledge directly in front of the tabernacle. If you return to church after the church has been locked you will have no choice but to visit the rectory and explain the situation. For this reason, it is best that you return to church not later than 12:45 PM on Sundays. Note that if there are only a very few hosts left you may consume them and purify the pyx at home and return the pyx on the next day it is convenient for you. Please do not repose empty ciboria or broken hosts. Broken hosts should be consumed. 32. Purify your pyx by going to the secrarum and opening the lid. Retrieve a purificator from the top edge of the basket (Don t reach inside. Just use the ones draped over the edge. If none are there, get a fresh one from the purificator drawer). Rinse the pyx and dry it with the purificator. Close the lid of the secrarum. EMs are not to purify ciboria or chalices or other sacred vessels. Deacons and priests do this. EMs must, however, purify their pyx before returning them to the pyx drawer. It is a sacrilege to allow particles of the Blessed Sacrament to remain in a pyx in a drawer. Your own personal pyx may be purified at home, taking care to pour the water into your garden and never into a sink. In this case, do not dry the pyx. 33. Please remember that you are Christophers in the truest sense of the word, and this is indeed a high honor. Listen to the Holy Spirit. He guides us in what we do and say, especially when carrying the Blessed Sacrament to the most helpless and vulnerable of Christians. You will often be amazed at yourself for the profound things you say, and the life-changing situations in which you will find yourself. Do not be surprised. You were God s means to an end. He planned it this way from the start. 4
